Objective: Data on the frequency of cardiovascular disease in people living with human immunodeficiency virus from low- and middle-income countries is scarce. Methods: We performed an observational study based on data from a historical cohort of people living with human immunodeficiency virus in Colombia during a 10-year follow-up to describe the prevalence of cardiovascular risk factors and their behavior according to CD4 count. Results: One thousand patients were initially included, out of which 390 had a 10-year follow-up. The mean age was 34 (standard deviation 10) years, and 90% were male. We observed an increase in the prevalence of dyslipidemia (29%-52%, p < 0.001) and obesity (1.1%-3.5%, p < 0.001). Major cardiovascular events occurred in less than 1% of patients. Patients with a CD4 count <200 cells/mm3 had a higher frequency of acute myocardial infarction and obesity. Conclusion: Over time, people living with human immunodeficiency virus present with an increasing prevalence of cardiovascular risk factors, particularly those with a lower CD4 count.

OBJECTIVE: We aimed to describe the prevalence and factors associated with the need for supplemental oxygen and persistent symptoms 1 year after severe SARS-CoV-2 infection. METHODS: In this historical cohort and nested case-control study, we included adults with severe COVID-19 (requiring admission to the intensive care unit or invasive mechanical ventilation). We evaluated factors associated with a need for supplemental oxygen and persistent symptoms 1 year after severe infection. RESULTS: We included 135 patients (median age 62 years, 30% women). At 1-year follow-up, the main symptoms were dyspnea (32%), myalgia (9%), cough (7%), anxiety (4%), and depression (5%); 12.59% of patients had prolonged requirement for supplemental oxygen. Factors associated with a persistent requirement for supplemental oxygen were female sex (odds ratio 3.15, 95% confidence interval 1.11-8.90) and Charlson Comorbidity Index > 4 (odds ratio 1.60, 95% confidence interval 1.20-2.12). CONCLUSIONS: We found that a high prevalence of supplemental oxygen requirement 1 year after severe COVID infection was associated with female sex and a baseline high rate of comorbidities. It is unknown whether this prevalence was related to other factors, such as the altitude at which patients lived. More than half of patients had prolonged post-COVID syndrome.

Desde la aparición de la enfermedad por SARS-CoV-2, han cambiado paradigmas en la ciencia médica y actualmente nos enfrentamos a las repercusiones a largo plazo que algunos de los pacientes desarrollan. Entre el grupo de pacientes que han cursado con neumonía severa-síndrome de dificultad respiratoria aguda (SDRA) por SARS-CoV-2 y que requirieron intubación orotraqueal (IOT), se documentan complicaciones multisistémicas (1-4). En este artículo destacaremos las complicaciones relacionadas con la vía aérea que se presentan en forma de estenosis benignas, siendo las lesiones subglóticas tipo simples o complejas las más frecuentes. A continuación, presentamos el caso de un paciente que presentó estenosis traqueales pos-IOT a causa de una neumonía severa por COVID-19 con diagnóstico, seguimiento y manejo por un grupo multidisciplinario de vía aérea.


Since the appearance of SARSCoV-2 disease, paradigms have changed in medical science, and we are currently facing the long-term repercussions that some of the patients develop. Within the group of patients who have had severe pneumonia - Acute respiratory distress syndrome (ARDS) due to SARSCoV-2 and who required orotracheal intubation (OTI), multisystemic complications are documented (1-4), in In this article, we will highlight airway-related complications that occur in the form of benign stenosis, with simple or complex subglottic lesions being the most common. We present a case of a patient who presented tracheal stenosis post-OTI due to severe COVID-19 pneumonia as main diagnosis, follow-up, and management by a multidisciplinary airway group.
